Required Proof of Income Documents

Type of Income
Document(s)
Wage or Salary
Submit the two most recent paystubs, not more than 90 calendar days old for each wage earning customer.
Paystubs should show your year-to-date earnings. If year-to-date earnings are not included on your paystubs, then also submit one of the following:
  • A letter from your employer stating your year-to-date earnings
  • Your two most recent bank statements income matching the paystubs
If you are a new employee and do not yet have a paystub, then submit a letter from your employer stating your hire date and salary.
Income from self-employment
Submit one of the following:
  • The most recent quarter’s Profit and Loss Statement
  • Year-to-Date Profit and Loss Statement covering at least a 3-month period

Benefit Income
(includes, but is not limited to, social security, disability, survivor benefits, pension, public assistance, adoption assistance)
Submit both of the following:
  • Award letter or benefit statement
  • Your two most recent monthly bank statements or copies of the two most recent benefit checks or two most recent monthly online statements for public assistance EBT card recipients
Food Stamps
Submit both of the following:
  • The award letter showing the amount of the benefit.
  • Your two most recent monthly bank statements or copies of the two most recent benefit checks or two most recent monthly online statements for public assistance EBT card recipients
Household income from a non-borrower
The non-borrower must reside in the property.
Submit one of the following:
  • Two consecutive paystubs or documentation of income matching the documentation requirements stated here for self-employment or other income sources, not more than 90 calendar days old
  • A Profit and Loss Statement and a bank statement
If the non-borrower is not contributing their full income, then the non-borrower must also submit a contribution letter showing the amount they are contributing on a monthly basis.
Alimony, Child Support, Separation Maintenance
Submit both of the following:
  • Copy of the divorce decree, separation agreement, other legal written agreement filed with a court, or a court decree that provides for the payment of alimony or child support and states the amount of the award and the period of time over which it will be received
  • Evidence of receipt of payment, such as copies of the two most recent bank statements or deposit advices showing deposit amounts
Please note that you are not required to disclose receipt of alimony, child support, or separation maintenance unless you want the income considered as part of your workout options review.
Rental Income
Submit the completed Schedule E from your most recent tax return.
If you did not file a Schedule E, then submit both of the following:
  • The lease agreement
  • Your two most recent bank statements showing the rental income received
Other Income
(includes, but is not limited to, bonus, commission, fee, housing allowance, tips and overtime)
Submit reliable third party documentation describing the nature of the income (e.g., an employment contract or printouts documenting tip income).
